It is almost five years since the referendum on Scottish independence. A lot has happened since then, but one thing that hasn't featured is an open and frank debate on why the 'yes' side lost. Nearly a year ago, a document was published by the SNP - the report of the Sustainable Growth Commission - set up by Nicola Sturgeon, to update and rethink the case for independence.
